In order to guarantee your student's spot for next year,

the following must be printed and dropped off at the Front Office for each student accepted for the 2019-2020 school year.

3.  Migrant Education Program Form

(Required form to be completed even if 'No' response)

4.  Health Information Form

5.  Student Residency Questionnaire

6.  Copy of your Child's Birth Certificate

7.  Copy of your Child's Immunization Record

8.  Copy of your Proof of Residence

     Please provide a copy of ONE of the following:       

    - Purchase Agreement 

    - Lease Agreement  

    - Warranty Deed 

    - Deed of Trust

- Tax Notice
